Often some assistance in the day to day household or personal tasks can mean the difference between being able to stay in your own home and having to move into an aged care facility.

TriCare’s Home Care packages (CACPS) assist our clients to remain in their own home as long as possible, maintain independence in a safe environment and allow them to stay active within their community.  We offer flexible packages to suit individual requirements and all the services we provide are reviewed and adjusted as our client’s needs change.

Our Services

Our friendly and professional team can provide assistance with:

• Light Housework
• Laundry
• Shopping
• Transport to local medical appointments or social activities
• Personal hygiene including;  showering and toileting
• Dressing
• Personal Grooming
• Preparation or supply of meals
• Review home safety, recommend and assist with arranging necessary modifications
• Respite, in home or at a residential facility

How to Apply for Assistance

You require an assessment to be conducted by an Aged Care Assessment Team (ACAT) to review your current situation and provide an individual client record of your current care needs.  Your local  Aged Care Assessment Team can be contacted directly or your GP can refer you.

Once you have received your assessment please contact your nearest TriCare Home Care Coordinator so we can make an appointment to design your personalised assistance package.
