In partnership with the Queensland government, TriCare offers transitional care at some of our residences. Transitional care is a program specially designed for those who require a level of hospital care in a home like setting before returning home.
Transitional care is provided on an individually assessed basis and is generally for a few weeks duration. It takes pressure off families, who can have peace of mind knowing that their loved one is receiving quality care in a home like environment.
Transitional care placements are handled directly by hospitals so if you think you or a relative are eligible, talk to your doctor or discharge planner at the hospital in question.
